The Divisions of Pulmonary and Critical Care Medicine at John H Stroger Hospital offer a fully accredited, competitive 3-year fellowship program in pulmonary and critical care medicine. The mission of our program is to train physicians to become highly competent specialists in the areas of pulmonary diseases and critical care medicine. We achieve this mission by instructing our fellows in a very diverse, rich clinical setting. We offer a wide broad didactic training and provide close personal supervision to the fellows by our teaching faculty.
All our fellows complete 27 months of clinical training which primarily occurs at the John H Stroger Hospital of Cook County, a public hospital serving the residents of the County of Cook. Our core rotations cover the following areas:
- Rotations in the Medical, cardiac, neurological, surgical care units: The hospital has 82 adult critical care beds divided between these six units. The medical intensive care unit has 22 beds. The fellows supervise medical students, interns, and residents, assess and triage admissions to the MICU, and lead the cardiopulmonary arrest team in the hospital. The fellows also learn to apply therapeutic hypothermia, bedside ultrasound, and the principles of hemodynamic monitoring and mechanical ventilation.
- Night float: the fellows cover the MICU during the night 5 night per week.
- Pulmonary consultation service: The pulmonary consult service performs an average of 1500 inpatient consults per year. During this rotation, the fellows are exposed to a wide variety of pulmonary pathology such as: pulmonary infections including a large number of patients with tuberculosis and opportunistic lung infections, obstructive lung diseases, interstitial lung diseases, pulmonary vascular diseases, thoracic malignancies, pleural diseases, neuromuscular diseases, and sleep disordered breathing. During this rotation, the fellows learn how to evaluate: pulmonary physiology tests, chest imaging, nuclear medicine images, pulmonary histopathology and microbiology.
- Procedures: The inpatient pulmonary consult service, the intensive care unit service and the pulmonary clinics provide the fellows with a vast opportunity to master pulmonary procedures. On average, each of our fellows performs 120 bronchoscopies, 35 intubations, 50 central lines, 5 chest tubes, 5 percutaneous tracheostomies, and 5 pleural biopsies.
- Pulmonary function testing: The fellows will learn how to interpret spirometry, lung volumes, arterial blood gases, bronchoprovocation challenge tests, and cardiopulmonary exercise tests.
- Sleep Medicine: The fellow learn how to evaluate patients with sleep disordered breathing as well as patients with other sleep disorders like narcolepsy, parasomnias, and insomnia. They also learn the basics of interpreting polysomnograms.
- Pulmonary Rehabilitation: The division runs a complete phase II outpatient pulmonary rehab program and offers phase III outpatient maintenance pulmonary rehabilitation. Our fellows learn how to screen the candidates for the program, how to write rehab prescriptions, and how to evaluate program outcomes.
- Outpatient Clinics: The outpatient clinical exposure in our program is vast. All fellows attend a half-day general pulmonary clinic for three years. In the second year, the fellows attend an additional half-day asthma clinic, and in the third year, they alternate between the sleep disorders clinic, pulmonary hypertension, and the pulmonary occupational diseases clinic.
The didactic content of our program is based in divisional and multi-disciplinary conferences. These conferences include: the core curriculum lectures, grand rounds, clinical case conference, radiology conference, journal club, morbidity and mortality, and research conference.
Every year we receive more than 250 applications of which we interview 40 for the 3 available positions. All applicants must use the ERAS system. A completed file must have: 3 current letters of recommendation, transcripts from medical school, ECFMG certificate, USMLE scores, a CV, a personal statement explaining why you are interested in Pulmonary and Critical Care and the ERAS application. The deadline for accepting applications is August 1. Incomplete applications will not be reviewed. We only offer positions to fellows through the national match system. At present, we support J-1 and H-1 visas.
