Oak Forest Hospital is a 900 bed healthcare facility located in the
southwest suburb of Oak Forest, Illinois. Services are available across
the continuum of care, including acute, subacute, skilled nursing, intermediate
care, comprehensive rehabilitation, and ambulatory care. The facility serves
the comprehensive rehabilitation care needs of patients referred from all
over Cook County and metropolitan Chicago. An After Hours Care Center was
opened to complement and extend primary care access for medically indigent
community residents and those in medically underserved communities of the
southwest suburbs. The on-site specialty ambulatory clinics provide medical
and surgical specialty care to patients referred from the community health
centers and primary care physicians who care for medically indigent patients.
Oak Forest Hospital maintains accreditation by the Joint Commission
on Accreditation of healthcare Organizations. The Comprehensive Rehabilitation
Program is accredited by the Commission on Accreditation of Rehabilitation
Facilities.
LEVELS OF SERVICE
Oak Forest Hospital is a unique health care center devoted to the diagnosis,
treatment, rehabilitation and long-term care of persons suffering from
chronic illnesses, degenerative diseases and physical impairments. Our
patients receive a total spectrum of services in order to increase their
level of independence, return them to the community, and enhance wellness.
Six levels of service are available through the facility.
Acute Hospital:
Patients admitted to the Acute Hospital require continuous management
of serious illnesses or acute phases of chronic disease. The Acute Hospital
consists of a medical/surgical unit and a critical care unit.
Comprehensive Rehabilitation Unit:
This program provides comprehensive rehabilitation services within the
unit, throughout other hospital areas, and on an outpatient basis.
Chronic Disease Hospital:
In this unit, patients require frequent medical, diagnostic, and therapeutic
services with continuous nursing supervision. Health care clinicians prepare
these patients for discharge back to the community.
Intermediate Care Facility:
Patients with long-term illnesses or disabilities which may have stabilized
and require only minimal medical supervision are served in this facility.
The treatment goal is to discharge patients to a private home or another
community residential setting.
Ambulatory Care:
Hospital outpatient clinics provide services in a variety of specialities.
Oak Forest Hospital recognizes that medical care extends beyond the impatient
stay.
Community Medical Centers:
Oak Forest Hospital managed three outpatient clinics in South suburban
Cook County. The Cottage Grove Medical Center, the Woody Winston Medical
Center, and the Lincoln Memorial Medical Center maintain and improve the
health of families through primary, speciality, and preventative services.
TEAM WORK
The progressive facility staffs more than 2,000 employees, including
physicians, nurses and highly trained health care technicians.
Patients receive 24-hour-a-day physician coverage. The medical staff
consists of board-certified physicians representing an extensive variety
of specialities.

VOLUNTEER CARE
This dedicated group from the community assists patients by supplying
reading materials from the Library, writing letters, assisting with recreational
activities, and lending a friendly ear.
FOLLOW-UP SERVICES
Once a person is released from inpatient care iyr health care professionals
ensure that proper follow-up care is available. This might include visits
to the hospital's outpatient clinic or to other resources in the community.
OUTPATIENT SERVICES
A large number of outpatient services are available to community residents.
Diagnostic services, therapy services, social services and treatment clinics
enable outpatients to receive cost effective and quality care,
ADMISSION INFORMATION
The facilities of Oak Forest Hospital are available to patients regardless
of their financial circumstances. Admission is made strictly of the basis
of madical condition. Admission applicants are reviewed by staff physicians
to assure placement in the level of service suited to a patient's individual
needs.
