| Before you can carry out research with humans or vertebrate animals
-- and before you apply for external funding for any kind of project --
certain internal reviews and approvals have to be completed. For a discussion
of the rationale behind these requirements, see Who
Signs for What and Why.
In all forms, the signatures you will need to obtain before submitting
your application are marked by a "@" sign.
In this section you can see the forms to be used for these internal
reviews and approvals. You can print them from your browser screen to fill
out with a typewriter, or you can download them in a number of electronic
formats. These allow you to fill out the forms on your computer
and print them directly. Forms are available in Wordperfect
and Microsoft Word for Windows. You can download them online by clicking
here
or you can pick up a disk copy from the Office of Research Development
Resource
Room.
To find out which forms you will need for particular types of projects,
see the Forms Checklist.
