Recommended Procedures for Requesting IRB Approval

On two pages (D-III.f), give the following information:

Cite the specific exemption criteria detailed in Guidelines for Research Investigators (Guidebook, page 4)—give number and letter.

Project title
-
PI name, address, phone, and fax.

Half-page description of project.

All other requests (expedited and full)
|
|
Deliver complete set of materials to Sihlali Funeka, IRB Quality Assurence Officer, Office of Research Affairs, Room 603, Durand Bldg. (312-864-4821). She leaves by 4:00 pm.

If she has time, she will quickly review set of materials while you wait to see if the materials are complete (using a checklist).

If the materials are not complete (based on her checklist), then she will return them by mail to the PI.

If they are complete, they are put on the agenda for the next meeting.

Recommendation: Call Ms. Funeka and schedule a time to hand deliver the materials so she will have time to review them while you wait.
|

Ms. Funeka arranges for reviewers and will tell you the date the proposal will be discussed during an IRB committee meeting. The IRB meets the first and third Tuesday of every month. Each meeting day is the deadline for the next meeting. Expedited reviews are done on a continuous basis. Ms. Funeka also writes the letter needed for grant submissions that states that the project is under review by the IRB. |

Any revisions or additions to the proposal need to be given to Ms. Funeka. They must be approved by expedited a full review.

Recommendation: Check with Ms. Funeka within 2 days of submitting the materials to determine the date of the formal IRB review.
